Look together, on a set rhythm, at the quality signals, the participant feedback, and whether delivery is holding to the standard, treating problems as information rather than blame.

Each cycle, look together at the quality signals, what the feedback is saying, and whether delivery is holding to the standard.

Where is delivery strong

Where is it slipping

What is the feedback telling us

The fidelity check. Has delivery drifted from the standard? For each drift, decide: a problem to correct, or a genuine improvement to fold back into the standard? Drift to fix, and improvement to adopt

The no-blame rule. A problem named here is information, not a failure of the person who named it. Is the review safe enough that people tell the truth in it? (yes / needs work)
