Map where the organization has fractured into silos and where information fails to flow, because the specific breakpoints decide the fix, and turf, distance, and bad habits each need a different response.

The silos: where do the parts of the organization fail to work together, and is it turf, distance, or habit

The information gaps: where does information get stuck or lost, so people are surprised by what they should have known

The dropped and duplicated work: what falls through the gaps between groups, or gets done twice

The buried conflict: where do disagreements go underground and come out sideways

The priority fractures, the ones costing the mission the most

The test: can staff who work across the boundaries confirm this map? The silos look different from inside each one, so a read from only the top will miss where the real walls are.
