Make every claim traceable to a complete, retained, findable record, because audit-readiness is what keeps money you were paid from being taken back.

The first step of protecting against clawbacks. For a sample of claims, confirm each traces to a record showing the service was delivered, to an eligible person, as documented, within the rules.

Traceable to a record (yes / no)

Complete against the contract (yes / no)

Retained for as long as the payer can look back (yes / no)

Organized so it can be found quickly (yes / no)

Close the gaps

Where records are missing, incomplete, or unfindable, close the gap going forward and note the exposure on past claims, so you know where you stand before anyone else looks. Gaps closed and past exposure noted
